Bone Marrow Edema syndrome in the Femoral head -A report of 4 cases-

Abstract
Purpose: To analyze the clinical manifestation and radiographic findings of bone marrow edema syndrome, and to differentiate this disease entity from the early stages of avascular necrosis of the femoral head.

material and Method: Four cases of bone marrow edema syndrome, diagnosed in our department, were reviewed. All the patients were male, with an average age of 40.8 years. Core decompressions & biopsies were performed due to failed conservative management. The preoperative clinical manifestations and radiographic findings were compared with the postoperative results, and the core biopsy finding and intraosseous pressure of the femoral head assessed.

Result: Preoperative magnetic resonance imaging showed edematous change of the femoral head, and bone scans showed hot uptake in the affected areas of the femoral head. The core biopsies of the femoral heads showed a yellowish fatty marrow pattern, with empty lacunae, and the intraosseous pressure increased to an average of 56.1 mmHg. Postoperatively, the clinical manifestations were improved and the Harris hip scores increased from 73.3 to 98 point 12 months postoperatively. Follow-up magnetic resonance examinations, following the core decompressions, showed normal findings in all cases.

Conclusion: Bone marrow edema syndrome should be differentiated from the early stages avascular necrosis for successful treatment results.
